Purpose
- To explain to Helpers how to use their Member’s Help Plan
How to use the Help Plan
- Before starting a shift with a Member, the Helper should read through the Member’s Help Plan to understand:
- The disability information related to the Member
- Who the emergency contacts are
- How the Member wants to be worked with
- If there are any things that Helpers should avoid doing
- How to deal with difficult situations and medical emergencies that the Member might have
- What the NDIS goals of the Member are, and what supports should be provided to the Member
- All Helpers should be familiar with their Member’s Help Plan, and check it regularly for any updates
- Forms, like the incident report form, RAD, and other important forms are linked within the Help Plan
- Helpers should message the Care Manager if they know of any updates or changes that should be made to a Help Plan
How to Access the Help Plan
The Help Plan can be found in the Lend A Friend rostering app, by following the steps below:
- Open the Lookout Way App
- Click on the “people” menu
- Select the Members name you want to search
- Select “Help Plan”
NOTE: The first part of the Help Plan labelled “Who I Am” contains links to important forms, like Incident reports, and the RAD
