Updated and Effective from: July 2024
Purpose
- To explain to Helpers how to use the Rates Agreement Document, which is the form used to capture how much they will charge for their disability services
What is the RAD?
- The Rates Agreement Document (RAD) is a form that captures the rates Helpers and Members negotiate for the Helper’s NDIS services
- Members have been told that they will be charged as per the up to date NDIS Pricing Arrangements if a RAD is not signed
- On the first meeting between a Helper and Member, the Helper should:
- Open the RAD form
- Discuss and fill out the relevant sections of the form
- Sign the form, and allow the Member to sign the form
- Once submitted, the Helpers rates will be added to the system so Lend A Friend can bill Members and pay Helper bills accordingly
How to Access the RAD form
The RAD form can be found in the Lend A Friend rostering app, by following the steps below:
- Open the Lookout Way App
- Click on the “people” menu
- Select the Members name you want to search
- Select “Help Plan”
- Select “Who I Am”
- Select “Rates Agreement Document (RAD)”
Filling out the RAD form
- The RAD form explains how it works within the form itself
