Your service agreement is a dynamic document and can be changed, revised, and terminated if and as needed.
Changes to the Service Agreement
- The Service Agreement is in effect from the date it is signed until the next NDIS plan review date, for the next 12 months, until superseded with a new service agreement, or until formally terminated
- Service Agreements and policy documents can be revised as needed by Lend A Friend Pty Ltd
- If large changes to services are required, participants can request a change to their Service Agreements by contacting the Care Manager listed in their Service Agreement and requesting a revision to their Service Agreement. A large change may involve:
- Changing the type of funding category being charged
- Drastically increasing or decreasing services
- Having a new NDIS plan which is very different to the plan which their Service Agreement is based on
- Where minor changes to a Service Agreement are desired, these changes can be made on an as needed basis without altering the Service Agreement, via negotiation between the Provider, Helpers and Participant. A minor change may involve:
- Slightly more or less hours of services requested
- Different activities or services requested which still fall under the same billing category
Termination of service agreement
- If the participant no longer wishes to receive services from Lend A Friend, they should contact their Care Manager listed in their Service Agreement, or the support team at Lend A Friend and make this known
- Ideally 2 weeks’ notice will be provided before termination of services so Lend A Friend can update plan managers, coordinators, and other involved parties of their cessation of services to the Participant. This it to ensure handover is seamless and any case notes or other tasks can be properly finalised
- Ideally, 2 weeks’ notice will be provided before terminating services, so Lend A Friend has the opportunity to find other suitable work for staff members and Helpers
- In the case of an immediate termination of services, the short notice cancellation policy still applies
