We will always discuss with a Participant their goals, services they require, and make sure that these services comply with the NDIS.

The services we will provide are listed in the service agreement under Schedule of Support and in the Schedule 1 table.

All the services listed are reasonable and necessary as defined by the National Disability Insurance Agency. The Participant agrees that each of the services provided by the Provider, as stated in Schedule 1 and Schedule of Supports, correspond directly or indirectly to the goals of the Participant.

To remain compliant, the Participant or Participant’s representative will immediately notify the Provider if the Participant’s NDIS Plan is replaced by a new plan or the Participant stops being a participant in the NDIS. This is grounds for amendment to the Service Agreement.

For the Purposes of Goods and Services Tax (GST) legislation, the Parties confirm that:

• the Participant has an active NDIS plan and the Participant’s NDIS plan is expected to remain in effect during the period the supports are provided;
• the supply of supports under this Service Agreement are covered by the Disability Services Minister’s determination for GST purposes, as specified in the Schedule 1 to the GST-free Supply (Care) Determination 2017; and
• all prices are GST inclusive (if applicable).
